Photography Class Interest Form

Should probably be mimicked from the Workshop Interest Form

Replace all workshop specific information with information from the photography class ideas.

Should be handed out at the beginning of kids/adult classes, so that parents/guardians can fill out the form, and bring back to byteworks on subsequent class dates when they drop kids off.
